(A) Manual correction method
Daily hot compresses are applied to the foot of the child, and the right hoof foot is used as an example, with the child lying flat on the bed and maintaining hip and knee flexion
position. The middle finger of the left hand is used to hold the top of both ankles, and the top of the left thumb is pressed against the metatarsal head of the lateral projection of the dorsum of the foot.
The right hand is used to hold the forefoot of the child. The operator’s right hand is used to dorsally extend the first metatarsal of the affected foot (placing the forefoot in a rotated posterior position) and then gently abduct the forefoot in a continuous manner. The left thumb is used as a fulcrum and the top pressure is applied to the metatarsals to counteract the effect. Perform the maneuver continuously for 10 minutes, rest for a moment, and then do it again, continuously
Perform 3~5 times.
(ii) Series of plaster cast fixation
After the manual correction and series of plaster fixation, Denis Brown orthopedic shoes should be worn throughout the day to consolidate the orthopedic effect. When the child is able to stand, he/she should wear the brace at night for at least 1 year to prevent recurrence of the deformity. Treatment by the Ponseti method is applied.
Indications.
Correction of various causes of foot pronation, internal and external rotation deformity of the foot, and spastic deformity of the pronator teres. For example: congenital clubfoot, internal and external rotation of the foot caused by cerebral palsy, scissor gait, etc. It is used for night correction and bedside correction.
